Keloids on the head are most often found across the nape, where the hairline meets the neck, and on the rear of the scalp. They usually start as folliculitis: ingrown hairs after a close skin fade become inflamed and infected, heal as small hard ‘shaving bumps’ and gradually merge into larger keloids, a condition known as acne keloidalis nuchae. Scalp keloids can also form after surgery for cysts and lipomas, or following cuts and injuries.
At Rejuvence Clinic, we treat scalp keloids of every grade, from early papules and nodules through to large keloid tumours and plaques. Every patient undergoes a thorough consultation so we can grade the condition, check for underlying infection and agree a treatment plan based on the size, location and behaviour of the keloid.
We offer both non-surgical (conservative) and surgical treatment options. Because scalp keloids are so often driven by trapped hairs and recurrent infection, treatment usually combines intralesional steroid injections with Nd:YAG laser to destroy the hair follicles within the scar, alongside antibiotics where needed. Where surgery is required, it is always followed by a structured programme of injections and laser to minimise the risk of the keloid returning.

Dr Sam Ghani is highly experienced in the surgical removal of scalp keloids, including large keloid tumours at the back of the head and complex or recurrent cases following previous treatment elsewhere. Removal is usually performed as a day procedure using local anaesthetic, with the incision positioned to protect the natural contour of the scalp and hairline.
Surgery is never offered in isolation. Every procedure is followed by a course of steroid injections into the scar line and, where ingrown hairs are the underlying cause, Nd:YAG laser to the surrounding area, supported by close post-operative follow-up. For patients who wish to camouflage the final scar, scalp micropigmentation can be considered once treatment is complete.

Do scalp keloids come back after treatment?
Recurrence rates on the scalp are relatively high, particularly where trapped hairs and infection remain. Combination treatment and avoiding close shaving reduce this risk, and we encourage patients to book a review the moment they notice new swelling or itching so it can be dealt with early. Read More.
