Facial and neck keloids are among the most visible and distressing forms of keloid scarring. They most often develop along the jawline, cheeks and chin, in the beard area and on the neck. The most common triggers are recurrent cystic acne, ingrown hairs and shaving bumps (folliculitis). Less often they follow piercings just in front of the ear, surgery or injury.
Facial keloids make up a significant part of our work at Rejuvence Clinic and we treat them in people of all skin types. Each consultation is used to pin down what is driving the keloid, because treating the scar alone without controlling active acne or ingrown hairs is far less effective.
Both non-surgical and surgical routes are available. First-line treatment is normally intralesional steroid injections, usually combined with Nd:YAG laser to soften and flatten the scar, improve its colour and destroy the follicles and oil glands that keep re-triggering it. Stubborn keloids may also benefit from 5-FU injections. Where surgery is required, it forms part of a wider prevention-led plan with post-operative injections and laser to minimise the risk of recurrence.

What measures can I take to prevent the keloids forming?
Keloids on the face and neck are particularly prone to recurrence because these areas are frequently affected by inflammation, friction, shaving and grooming, as well as skin conditions such as acne and folliculitis. Achieving the best long-term results therefore involves more than simply removing the keloid. Effective treatment should also address the underlying causes of inflammation and minimise ongoing skin irritation, helping to reduce the risk of the keloid returning. Read More.
Why do keloids form on the face and neck?
The usual culprit is repeated cystic acne: spots become inflamed, rupture and heal as thickened keloid tissue, typically along the jawline and cheeks. Ingrown hairs and shaving bumps in the beard area and neck have the same effect, and piercings in front of the ear, facial surgery and injuries can also result in keloids.
Who is most likely to develop facial keloids?
Keloids are more common in people of Afro-Caribbean, South Asian and Southeast Asian heritage. Men with thick, curly beard hair who shave closely are particularly prone, as are women with excess hair growth along the jawline, chin and neck.
Do face and neck keloids come back?
The face and neck are shaved, touched, rubbed by collars and prone to breakouts, so the risk of recurrence is higher here than elsewhere. Long-term planning, addressing the underlying trigger and early review of any new swelling all help to keep keloids under control.
